By purchasing or using this voucher, you agree to the following terms and conditions:

1. Voucher Eligibility

Valid for new patients only
Valid for one person and one visit only
Must be redeemed at Petone Dental Centre

2. What’s Included

The voucher includes:

Dental check-up
Up to 2 standard x-rays (if clinically required)
Scale, clean, and polish
Treatment planning discussion (if needed)

Any additional treatment is not included and will be discussed separately.

3. Booking Requirements

Booking is essential and subject to availability
Must be redeemed within 90 days of purchase unless otherwise stated

4. Cancellations & Missed Appointments

A minimum of 48 hours’ notice is required for cancellations or rescheduling
